Salon Partner Checklist: Documents & Steps

How to Complete the Partner Intake

1) Business Info

• Legal business name and DBA

• EIN or Tax ID

• Primary contact (name, title, email, mobile)

• Billing and shipping addresses

2) Required Upload

• Salon, Business, or Cosmetology License (image or PDF)

3) Brand and Marketing

• Premium brands currently carried.

• Instagram, Facebook, TikTok, and website

• Salon logo (PNG or SVG, optional)

• Private fitting room available (Yes or No)

4) Practice Profile​

• Number of stylists in your salon

5) CPIS-307 Group Enrollment Interest

• Number of stylists you plan to certify

• Preferred cohort (June 2026 standard rate, or Spring 2027 early bird rate)

• Preferred curriculum call window

6) Payments and Terms

• Preferred method: ACH (preferred), Bank Wire, Card (prepay only), or Check

• Cards: prepay only, disclosed convenience fee may apply where permitted

• Checks: orders ship 3 to 5 business days after funds clear

• Terms: Prepay is the default. Net 15 available upon approval. Net 30 reserved for established partners.

• No COD

7) Required Acknowledgments

Check each to agree:

• Confidentiality. Base architectures, materials science, training content, and partner documentation are confidential. No public or social sharing.

• No-Marketplace Resale. No sales through Amazon, eBay, Etsy, Poshmark, or Facebook Marketplace. No rebranding without written consent.

• MAP and Brand Language. Use approved product names and materials language. No medical claims. Follow MAP pricing integrity.

• Sanitation and Safety. Patch test when adhesives are used. Single-use liners and tapes. No application on broken skin.

• Warranty and PCCA. Warranty evaluations follow the PCCA (Cap Comprehensive Assessment) process.

• Client Data and Privacy. Protect client measurements and photographs. Follow HIPAA-aligned practices where applicable. No sharing without client consent.

8) Signature

• Signer's full name and title

• Date

• Signature (typed name accepted if the signature field is unavailable)

9) Submit

• Submit to create your partner profile

• We confirm receipt within one business day

• We schedule your curriculum call and partnership review

• Approved partners receive next-step documents (W-9, Resale Certificate request, COI request) along with CPIS-307 cohort enrollment instructions
